Chronic Prostatitis Symptoms

Chronic prostatitis has varying symptoms. A lot of patients with a mild infection show no symptoms. The troubles which may occur are urinary or genital, accompanied by unpleasant sensations, or even pain. These sensations are localized inside the perineum, the rectum and the urethra and irradiate towards the ribs and penis. They start off with a sensation of weight or warmness, and sometimes with real pains.

Chronic prostatitis symptoms

Urinary troubles include:
  • the need to urinate frequently and urgently, that is the need to urinate needs to be satisfied immediately. This polachiuria ceases during the night, while sleeping.
  • latency in the urine flow and a weakening of it
  • rarely there incomplete or even complete retention of urine.
  • hematuria (blood in urine)
  • incontinence

All these troubles are caused by the increase in volume of the prostate gland and also by the inflammation of the urethra. We also notice the appearance of watery morning drip which is watery, clear or milky and sometimes, clammy. It shows in the morning, at meatus, after waking or even in the afternoon, if the person has not urinated in a long time (4-5 hours).

Genital troubles are always present and are among the following:
  • repeated and persistent erections, mostly nocturnal, and frequent nocturnal pollutions. These erections strain the ill.
  • in other cases there is a decrease in libido and erections, even their total disappearance, and true impotence occurs
  • during sexual intercourse there is hyper-excitability which precipitates the ejaculation, sometimes even at first contact.
  • smetimes the sexual intercourse calms down the sensations which trouble the ill, other times it aggravates them.
  • nervous troubles occur quite rapidly, sometimes very acute ones: moral depression, concerns about their impotence, their inability to get married, of the weakening which they attribute to their leaks (which they consider to be seminal leakage), etc. The general state can rapidly aggravate and contributes to the aggravation of the disease.
  • testicular pain
  • pain with ejaculation

Local symptoms can suffer periods of relief or can intensify, based on the strain, the sexual excesses, because the correct diet is not followed, coldness, constipation etc. One of the symptoms over which ill persons draw attention is prostatic fluid leakage, which occurs either when they empty the bowels, sometimes at the end of miction: this is called prostatorrhea and can be more or less abundant. Prostatorrhea is not always present in chronic prostatitis. A rectal tact can also provoke such leakage. The urine of a patient who has chronic prostatitis contain fibers, just like that of the chronic posterior urethritis patient.

The symptoms of seminal vesiculitis, associated with those of prostatitis, are similar to those of chronic prostatitis, but there is a predominance of sexual symptoms, such as frequent noturnal pollutions and premature and sanguinolent ejaculations. Nervous troubles are also frequent: headaches, dizziness, neurasthenia (shortly, it comes from neuro=nerve, asthenia="fatigue"). Repeated recurrences of Epididymitis incicate the chronic infection of the seminal vesicles, as a focal of infection.

Chronic prostatitis symptoms are have a continual and tenacious evolution, they frequently recur. Chronic prostatitis is the true cause in many cases of cystitis and urethritis. It can lead to sterility, through lesions of the ejaculatory ducts, vesiculitis and epididymitis. It can also evolve, year after year, without symptoms or with limited symptoms. It never cures spontaneously and very rarely with classical treatment (antibiotics). Chronic prostatitis maintains a state of infection in the urinary apparatus which can cause -every now and then- accesses of pielitis, pielonefritis, recurrent epididymitis etc. Through the local irritation it maintains, chronic prostatitis triggers a sclerosis of the bladder peak, with late mechanical reaction on the superior urinary apparatus.

The diagnostics of chronic prostatitis is established on basis of the existence of the signs and symptoms previously described, along with the clinical exam (DRE- Digital Rectal Exam), lab exam (urine samples- more info on prostatitis tests & lab analysis), and when required, xrays.
